Documents Required for the Active Duty Military Application
These are required documents for this property. Please review the list as you are preparing to submit your application.
STEP 1: Gather the Documents Needed for Your Application
- All applicants must provide one of the following:
- Orders
- RIP
- Memo
- All applicants must present a current Leave Earning Statement (LES) to confirm current rank.
- If all dependents (spouse/kids) are not listed on orders, you must provide the following documents:
- Marriage certificate or DD1172
- Birth certificates for every person under 18
- *If all dependents are listed on your orders, no additional documents are needed.
- If you have pets, you must provide all of the following:
- Shot records of each pet
- Photo of each pet
- If you are filing the information on behalf of an active-duty member, you need a power of attorney (POA).
- If you are dual-military, you must provide the following:
- Orders for both members
- Marriage certificate or DD1172 if one family member does not appear on the other's orders
STEP 2: Complete the HMC Application
After gathering the required documents, complete the HMC application by clicking apply now.
*Any individual 18 years of age or older who will be residing in the home but is not a dependent must also complete an application and note on the application with whom they will be residing. If they are not active duty, they must select the non-active duty application. Non-dependent applicants must pay an application fee via www.RentPayment.com to complete their application.
STEP 3: Complete Your MHO Referral

Next, you will need to contact the Military Housing Office (MHO) at Dyess AFB for a referral. You can reach them by phone at (325) 696-2150 or email at DyessHousingCenter@us.af.mil.
The MHO may require additional documents and forms to be completed in order to obtain a referral for military housing.
STEP 4: Waitlist Placement and Move-In Requirements
Once you have completed the above steps, you will be placed on a waitlist for homes that you qualify for until you are offered a home. You will receive confirmation once you are placed on the waitlist.
The following additional documents will be required at move-in:
- State-issued ID for every person in the household who is 18 or older.
- Account number for electric and gas service in your name.
- Proof of Liability or Renter's Insurance: Insurance information will be provided to you after you accept your home offer.
